Qualifications

• Active and unrestricted RN licensure in home state (CA, etc)
• 3+ years clinical experience (diabetes, CHF, CKD, post-acute, hospice, palliative, cardiac, home health) with Medicare members
• 2+ years case management, discharge planning, or home health coordination
• Proficiency with Word, Excel, Outlook, PowerPoint and proprietary applications
• Ability to work in fast-paced, high-volume environments and manage time effectively
• Strong medical terminology and digital literacy understanding
• Ability to handle sensitive information ethically and responsibly

Responsibilities

• Coordinate and facilitate comprehensive care for individuals
