Job summary

CVS Health is seeking a remote RN Case Manager to support members across multiple states. You will work telephonically with patients, apply clinical guidelines, and collaborate with care teams to optimize benefits and outcomes.

Requirements include 5+ years as an RN with hospital experience, multi-state license, and oncology background; bilingual Spanish is a plus. Full-time role with comprehensive benefits and 40 hours weekly.

Qualifications

• 5+ years as a Registered Nurse with at least 1 year in a hospital setting.
• Nurse must hold an active multistate license via the Nurse Licensure Compact (NLC) to practice across participating states.
• 1+ years' experience documenting electronically.
• 1+ years' Oncology experience.

Responsibilities

• Provide telephonic case management for fully and/or self-insured clients.
• Assess benefits and member needs against criteria, guidelines, and procedures to ensure proper administration of benefits.
• Apply clinical judgment to reduce risk factors and barriers in care planning and issue resolution.
• Assess all conditions, including co-morbidities, affecting functionality.
• Consult with supervisors and teams to overcome barriers and meet goals; participate in case conferences.
