Job summary

AltaMed Health Services Corp. seeks a Registered Nurse, Out-Patient Case Manager in Montebello, CA to coordinate care for patients with chronic diseases.

The RN Case Manager will lead a team, perform assessments, and collaborate with physicians and social services to support ambulatory and hospital-based care plans. Requirements include an accredited nursing program graduate, CA RN license, 3+ years in public health nursing/case management, bilingual in English/Spanish preferred, and a valid BLS

Qualifications

• Graduation from an accredited nursing program.
• Current valid RN license (CA).
• Experience in and willingness to work in a multi-disciplinary team.
• Experience with adults with physical or mental impairments; geriatric population preferred.

Responsibilities

• Provide daily care coordination, case management, coaching, and intervention for patients with chronic diseases.
• Lead a cohort of LVN Case Managers, Case Manager Coordinators, and Social Workers.
• Collaborate with interdisciplinary care team and physicians in the clinic.
• Perform telephonic and face-to-face assessments and coordinate services across care settings.
