Job summary

AltaMed Health Services is hiring an RN Case Manager for outpatient and ambulatory care coordination in Montebello, CA. You will lead the care management team, coordinate with physicians and social services, and provide telephonic and in-person assessments for patients with chronic conditions.

The role supports a diverse patient population, emphasizing equity and quality care. Requires California RN licensure, a multi-disciplinary approach, and bilingual English/Spanish preferred.

Qualifications

• Graduation from an accredited nursing program.
• Current RN license in California; social work or health-related degree with licensure preferred.
• Experience in a multi-disciplinary team.
• Experience with adults who are physically or mentally impaired and/or geriatric population.
• Three years of RN experience in public health nursing, acute care, case management, or home health care; ≥2 years in managed care.
• Bilingual in English and Spanish preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide daily care coordination and case management for patients with one or more chronic diseases.
• Lead a cohort of LVN Case Managers, Case Manager Coordinators, and Social Workers.
• Coordinate with interdisciplinary care team and physicians to support ambulatory and inpatient case management.
