Job summary

AltaMed Health Services Corp. is seeking a Registered Nurse, Out-Patient Case Manager to coordinate care for patients with chronic conditions in a multidisciplinary team. You will perform assessments, develop care plans, and connect patients with social work, mental health, home health, and community resources.

The role emphasizes leadership of a cohort of LVN Case Managers and Case Manager Coordinators, with telephonic and in-person interactions to improve outcomes and patient experience.

Qualifications

• Graduation from an accredited nursing program.
• Current valid RN license in California.
• Experience in and willingness to be part of a multi-disciplinary team.
• Experience with physically or mentally impaired adults and/or the geriatric population.
• Three years of RN experience in public health nursing, acute care, case management, and/or home health care; minimum 2 years in managed care.
• Care experience in case management with inpatient/outpatient ambulatory focus preferred.
• Bilingual in English and Spanish preferred.
• A minimum requirement of a valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide daily care coordination, case management, coaching, consultation, and intervention for patients with chronic diseases.
• Identify patient populations via referrals and registries and coordinate with the interdisciplinary care team.
• Lead a cohort of LVN Case Managers, Case Manager Coordinators, and Social Workers in case management activities.

Education

California RN license (registered nurse)

Bachelor's degree in nursing or related health field
