CVS Health Corporation seeks a Case Manager RN with a Compact License for a fully remote position. The role supports Community Care members nationwide, collaborating with providers, insurers, and families to coordinate comprehensive case management and discharge planning.

40 hours weekly, primarily Mon-Fri, 8:00 AM–5:00 PM ET, with travel up to 10% as required. Ideal candidates have 3+ years of clinical practice and 2+ years in case management or home health coordination, plus strong

• Active and unrestricted RN license (Compact) required.
• 3+ years clinical practice experience.
• 2+ years case management, discharge planning, or home health care coordination.
• Ability to travel up to 10% for in-person activities.

Responsibilities

• Act as liaison with members, providers, and insurers to manage care.
• Coordinate case management across acute and chronic conditions.
• Assess medical and vocational status; develop care plans and determine benefits eligibility.
• Communicate with stakeholders via phone or in-person and document activities.
