Job summary

CVS Health seeks an RN Case Manager for a 100% remote role; candidates must reside in the Eastern Time Zone. Preference for Compact RN licensure and Massachusetts residency. Typical schedule is Monday–Friday, 8am–5pm in residence time zone, with flexible hours to meet member needs.

The role includes telephonic and in-person assessments, care planning, coordination of services and utilization review to support member wellness. Travel is limited.

Qualifications

• Active RN license in state of residence; compact preferred.
• 3+ years acute care RN experience (medical, post-surgical, ICU, case management).
• Willingness to obtain additional state licenses (company-paid).
• EST Time Zone residency required.

Responsibilities

• Assess, plan, implement and coordinate all case management activities.
• Telephonically and face-to-face evaluation of member needs and benefits.
• Collaborate to refer to plan sponsor programs and resources.
• Apply clinical judgment to reduce risk factors and health issues.
