Job summary

BCBST BlueCross BlueShield of Tennessee, Inc. seeks an RN Case Manager to join the Population Health team in a fully remote capacity.

You will conduct telephonic outreach, assess member needs, plan and implement care, and coordinate with providers to ensure effective benefits delivery and positive health outcomes. You will educate members, empower self-care, and leverage AI-enabled tools to improve efficiency.

Qualifications

• Excellent oral and written communication skills.
• PC skills required (Basic Microsoft Office and E-Mail).
• Digital literacy assessment may be required.

Responsibilities

• Support utilization management functions for complex cases as needed.
• Serve as liaison between members, providers and internal/external customers in coordinating health care delivery and benefits programs.
• Oversee highly complex cases to ensure effective interventions and efficient utilization of benefits.
• Perform case management activities: assessment, planning, implementation, coordinating, monitoring, outcomes and evaluation.
