Job description

Join our Population Health team as a remote Social Work Case Manager and make a meaningful impact in the lives of elderly and disabled members. Through telephonic outreach, care coordination, advocacy, and connecting members to community and healthcare resources, you will support members with complex medical, behavioral, and social needs while empowering them to achieve their goals. In this role, you will partner with members, providers, caregivers, and community organizations to address barriers to care and improve health outcomes. To be successful in this role, you must hold an active Tennessee Social Work license and demonstrate strong communication, organization, and care coordination skills. We are particularly interested in candidates with experience in skilled nursing facilities (SNF), acute inpatient settings, discharge planning, community resource coordination, or hospice care. Note: This is a remote position with flexible scheduling available within business hours of 8:00 a.m. to 6:00 p.m. EST. CCM certification is preferred but not required. Job Responsibilities
  • Providing crisis intervention and helping families to understand the complexities and impact of the medical situation.
  • Assessing patient and family's needs for, and responses to, healthcare services on an ongoing basis.
  • Interviewing patients and planning activities to meet the social and emotional needs of them and their families, and to facilitate the recovery psychologically, emotionally and physically.
  • Coordinating with external agencies and home care nursing group for post-discharge care.
  • Must be able to perform home visits if necessary.
Job Qualifications
  • License Clinical Licensure or Education Master's degree required and one of the following unrestricted TN licensures: LCSW or LMSW.
  • Valid Driver's License required.
  • Experience 3 years - Experience providing care coordination to persons receiving long-term care and/or home and community based services required.
  • Various immunizations and/or associated medical tests may be required for this position.

  • Must be detailed oriented, deal well with ambiguity, demonstrate strong strategic thinking and creativity skills, be an effective communicator with both internal and external partners/customers Awareness of the impact of cultural/socioeconomic status pertaining to the use of healthcare and mental health care services.
  • Employees who are required to operate either a BCBST-owned vehicle or a personal or rental vehicle for company business on a routine basis* will be automatically enrolled into the BCBST Driver Safety Program. The employee will also be required to adhere to the guidelines set forth through the program. This includes, maintaining a valid driver’s license, auto insurance compliance with minimum liability requirements; as defined in the "Use of Non BCBST-Owned Vehicle" Policy (for employees driving personal or rental vehicles only); and maintaining an acceptable motor vehicle record (MVR).
