Job summary

CVS Health is seeking a Case Management Registered Nurse to collaborate with physicians, case managers and members to optimize care and outcomes. The role involves scheduling, benefits access and education support using care management tools and resources.

You will contribute to integrated care for Medicare/Medicaid populations while working primarily from a home-based or hybrid setting. The ideal candidate holds an Illinois RN license, 3–5 years of clinical experience, strong communication,

Qualifications

Illinois resident required.
Willing and able to travel up to 75% in Metro East/Southern Illinois; mileage reimbursed.
Minimum 3–5 years clinical experience.
Independent thinker who collaborates virtually.
Strong analytical and problem-solving skills.
Effective communication, organization and interpersonal abilities.
Proficient with MS Word, Excel, Outlook and PowerPoint; some proprietary apps.

Responsibilities

Collaborate to inform the case management process and facilitate outcomes.
Assist with appointment scheduling and access to benefits and education for members.
Utilize care management tools and resources to support members' health and social needs.
