Job summary
CVS Health is seeking a Case Management Registered Nurse to collaborate with members, providers, and care teams to optimize outcomes. This role focuses on scheduling appointments, facilitating access to benefits, and educating members using care management tools.
The ideal candidate will reside in Illinois, have 3–5 years of clinical experience, be comfortable working from home and traveling up to 75% in Southern Illinois, and demonstrate strong communication, analytical, and problem-solving
Qualifications
- Active Illinois RN license required.
- 3–5 years of clinical experience.
- Ability to travel up to 75% from home location in and around Metro East/Southern Illinois.
- Strong analytical, problem-solving and communication skills.
- Proficient with MS Word, Excel, Outlook, PowerPoint and EHR systems.
Responsibilities
- Collaborate with members, providers and care teams to inform the case management process.
- Assist with appointment scheduling and benefit access for members.
- Educate members using care management tools and resources.
- Navigate multiple systems to support independent work and care coordination.
