Job summary

CVS Health Corporation seeks a Case Management Registered Nurse to support the case management process for Medicare/Medicaid members in Illinois. The role involves coordinating appointments, helping members access benefits, and educating them using care management tools and resources.

The ideal candidate has 3–5 years of clinical experience, strong analytical and communication skills, and comfort working remotely.

Qualifications

Must reside in the state of Illinois.
Must possess reliable transportation and be willing and able to travel up to 75% of the time from home location in and around Metro East/Southern Illinois (Madison, St. Clair, Monroe, Clinton, Jersey, Calhoun, Bond, Macoupin and surrounding areas). Mileage is reimbursed per our company expense reimbursement policy
Minimum 3-5 years clinical practical experience
Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
Excellent analytical and problem‑solving skills
Effective communications, organizational, and interpersonal skills.
Ability to work independently
Effective computer skills including navigating multiple systems and keyboarding
Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications

Responsibilities

Facilitates case management processes and supports healthcare outcomes for members.
Assists with appointment scheduling and accessing benefits and education for members.
Collaborates with members, providers, and community organizations to address health and social determinants.
