Sentara Health Plans is seeking an Integrated Nurse Case Manager/Registered Nurse (RN) for chronic/complex conditions to serve Fredericksburg, Spotsylvania, VA, and surrounding areas. This full-time role is work-from-home with travel to member homes about 2–3 times per week.

Primary responsibilities include case management, assessments, and coordination of care across the health team.

Formación

• Associate or Bachelors Degree in Nursing REQUIRED.
• RN license (Compact or Virginia) REQUIRED.
• 3 years of nursing experience REQUIRED.
• Case management experience highly preferred.
• Experience with chronic/complex condition members and home health/care transition is preferred.

Responsabilidades

• Develop, monitor, evaluate, and revise the member care plan to optimize health across the continuum.
• Perform telephonic or face-to-face assessments to identify and manage member needs including physical, behavioral health, social services and LTC.
• Identify high-risk members and coordinate care with the health team.
• Manage chronic illnesses and comorbidities to ensure cost-effective care utilization.
• Facilitate authorizations/referrals within benefits structure and assist with care plan implementation.
