Job summary

Sentara Health Plans is seeking an Integrated Nurse Case Manager (RN) for Chronic/Complex Conditions to serve Fredericksburg, VA and surrounding areas. This full-time, remote role requires travel for face-to-face home visits 2–3 times a week.

Responsibilities include developing care plans, conducting assessments, coordinating with the care team, and ensuring quality and regulatory compliance. Must have RN license, 3 years nursing experience, and reside in the Fredericksburg area.

Qualifications

• Associate or Bachelors Degree in Nursing REQUIRED.
• Active RN license (Compact or VA) REQUIRED.
• 3+ years nursing experience REQUIRED.
• Case management experience highly preferred.

Responsibilities

• Develop, monitor, and revise care plans to optimize member health.
• Perform telephonic or in-person assessments for chronic/complex conditions.
• Coordinate with multidisciplinary teams to manage high-risk members.
• Navigate authorizations/referrals within benefits structures as needed.
• Ensure compliance with regulatory and company policies.
