Sentara Health Plans is hiring an Integrated Nurse Case Manager (RN) for chronic/complex conditions in Fredericksburg, VA and surrounding areas. This is a full-time, remote position requiring regular face-to-face home visits (2–3 times weekly) with travel and flexible schedule within 8am–5pm EST.

The role focuses on developing and monitoring care plans, coordinating medical and social services, and working with interdisciplinary teams to optimize member outcomes.

Qualifications

• Associate or Bachelor’s Degree in Nursing required.
• Registered Nurse (RN) license (Compact or Virginia) required.
• 3+ years of nursing experience required.

Responsabilités

• Develop, monitor, and revise the member care plan to optimize health across the care continuum.
• Perform telephonic or face-to-face assessments for needs assessment and care coordination.
• Identify high-risk members and coordinate care with the health team.
• Manage chronic illnesses to ensure cost-effective utilization of benefits.
• Facilitate authorizations/referrals within benefits structure as applicable.
• Interface with Medical Directors and care teams on treatment plans.
• Present cases at case conferences; ensure regulatory compliance.
• Assist with problem solving on provider or claims issues.
