Job summary

Sentara Health is seeking an Integrated Case Manager, RN. The position is remote but requires in-person face-to-face assessments with travel to Roanoke and northern areas up to 60-90 minutes, around 50-75% of the week.

You will develop and monitor care plans, perform telephonic assessments, and coordinate with the health care team to optimize member health across the care continuum. Requirements include an active RN license in Virginia and at least 3 years of nursing experience, with discharge

Qualifications

• Active RN license in Virginia
• 3 years of nursing experience required
• Discharge planning experience preferred
• Willingness to travel up to 60-90 minutes weekly to see members

Responsibilities

• Perform telephonic clinical assessments for care coordination and management of member needs.
• Develop and monitor member care plans to optimize health across the care continuum.
• Identify high-risk members and coordinate care with the health care team.
• Assist with authorizations/referrals within benefits structure.
• Interface with Medical Directors and interdisciplinary teams on care plans.
• Present cases at case conferences for multidisciplinary focus.
• Ensure compliance with regulatory, accrediting and company policies.
• Support problem solving with provider, claims or service issues.
• Address physical, psychological, socio-cultural, spiritual and cognitive needs of patients.
