Sentara Health Plans in Fredericksburg, VA is seeking an Integrated Nurse Case Manager (RN) for chronic/complex conditions. This full-time remote role requires 2–3 face-to-face home visits weekly, traveling to Fredericksburg and surrounding areas, with standard hours 8am-5pm EST, Monday through Friday.

Responsibilities include developing and monitoring care plans, conducting assessments, coordinating with the care team, and facilitating referrals.

Qualifications

• Associate or Bachelor’s Degree in Nursing required.
• Registered Nurse (RN) license (Compact or Virginia) required.
• Minimum 3 years of nursing experience with case management preferred.
• Experience with chronic/complex conditions and home health is highly preferred.

Responsabilités

• Develop, monitor, and revise member care plans to optimize health outcomes.
• Perform telephonic or face-to-face assessments for care coordination.
• Identify high-risk members and coordinate with care teams.
• Manage chronic illnesses and coordinate authorizations/referrals within benefits.
• Interface with Medical Directors and present cases at case conferences.
