Sentara Health in Harrisonburg, VA is seeking an Integrated Case Manager, RN to join our care management team. This is a remote position with travel required within Harrisonburg and surrounding areas.

The role focuses on developing, monitoring, and revising care plans to optimize health outcomes across the care continuum. The ideal candidate will have a registered nurse license, at least 3 years of nursing experience, and experience in managed care or discharge planning.

Formación

• Requires RN license and 3 years nursing experience; managed care and discharge planning experience preferred.
• Strong communication and analytical skills essential; ability to coordinate across disciplines.
• Knowledge of regulatory and policy compliance within case management and utilization management.

Responsabilidades

• Develop, monitor, evaluate, and revise member care plans to optimize health across the continuum.
• Conduct telephonic or in-person assessments for identification, coordination, and management of member needs.
• Coordinate authorizations/referrals within benefits or extra-contractual arrangements.
• Present cases at multidisciplinary case conferences and interface with Medical Directors and care teams.
• Ensure compliance with regulatory, accrediting, and company policies; resolve provider or service issues.
