Responsibilities

• Administers and coordinates care plans across settings to ensure smooth transitions.
• Analyzes complex patient data from medical history, tests, and treatment plans.
• Organizes patient files for easy retrieval using case management practices.
• Communicates securely through internal platforms for collaboration on care plans.
• Conducts routine utilization reviews to ensure appropriate, cost-effective care.
• Configures case management systems to track disease management and utilization review.
• Coordinates analytics projects to generate reports on health indicators.
• Designs processes to coordinate discharge planning from hospital to home.
• Develops resource management to optimize healthcare using community resources.
