Job summary

UnitedHealth Group is seeking a Remote RN Case Manager to coordinate transitions of care for ED discharges and collaborate with hospital and outpatient teams across the care continuum.

The role requires an active Multistate RN license, 4+ years of clinical experience, and ED exposure. You will support proactive outreach, documentation, and adherence to HIPAA while working from anywhere in the United States.

Qualifications

• Current, unrestricted Multistate RN license.
• 4+ years of diverse clinical experience with acutely ill patients.
• 1+ years experience in the Emergency Department.
• Knowledge of utilization management, quality improvement, and discharge planning.
• Rapid acuity-based clinical prioritization.
• Recognizing early signs of clinical deterioration.
• Identifying patients at risk for ED return visits or avoidable bounce-backs.
• Credibility with ED physicians and staff.
• Understanding ED workflow, throughput, and provider decision-making.

Responsibilities

• Collaborates with ED and outpatient care teams to establish an individualized transition plan for members.
• Serves as the clinical liaison with hospital staff and coordinates transition of care for ED discharges.
• Performs clinical reviews (expedited, standard, concurrent, retrospective) for in-network/out-of-network facilities.
• Communicates discharge plans and health plan benefits with members and families.
• Monitors real-time ADT feeds to assess discharge opportunities and coordinate care.
• Conducts transition of care outreach after ED discharge to confirm access and completion of plans.
• Manages caseload efficiently and adheres to time management practices.
• Maintains HIPAA-compliant documentation and audit scores.
• Maintains licensure and hospital credentialing as required.
