Qualifications

• Current, unrestricted Multistate RN license.
• 4+ years of diverse clinical experience in caring for acutely ill patients.
• 1+ year experience in the Emergency Department.
• Knowledge of utilization management, quality improvement, and discharge planning.

Responsibilities

• Collaborate with ED and outpatient care teams to establish an individualized transition plan.
• Serve as clinical liaison with hospital staff to coordinate discharge planning.
• Perform clinical reviews across in-network and out-of-network facilities.
• Communicate discharge plans and health plan benefits to members and families.
