Job summary

UnitedHealth Group is seeking a RN Case Manager to monitor real-time ADT notifications and coordinate proactive discharge planning for members, enabling smooth transitions of care across ED and outpatient settings.

The role supports flexible, remote work from anywhere in the U.S. and requires an active RN license (multistate) with at least 4 years of clinical experience, including ED exposure.

Qualifications

• Current, unrestricted Multistate RN license.
• 4+ years of diverse clinical experience in caring for acutely ill patients.
• 1+ years of experience in the Emergency Department.
• Knowledge of utilization management, quality improvement, and discharge planning.
• Rapid acuity-based clinical prioritization.

Responsibilities

• Collaborate with ED and outpatient teams to create individualized transition plans.
• Serve as clinical liaison and perform transition of care for ED discharges.
• Conduct various clinical reviews (expedited, standard, concurrent, retrospective).
• Communicate discharge needs and plan with ED staff, members, and families.
• Monitor real-time ADT feeds to identify discharge opportunities and risks.
• Conduct transition of care outreach to confirm discharge plan completion.
• Manage caseload efficiently and maintain documentation for audits.
