Job summary
CVS Health Corporation is seeking a dedicated Registered Nurse to join our Aetna team in Illinois as an on-site Case Manager. This role focuses on coordinating care for dual-eligible members, assessing needs, promoting wellness, and facilitating return-to-work across Kane, LaSalle, and surrounding counties.
The ideal candidate holds an Associate's or Bachelor's in Nursing, 3+ years clinical experience, and is comfortable with 50–75% travel.
Qualifications
- 3+ years clinical practical experience
- Reside in Kane, LaSalle or surrounding counties
- Willing to travel up to 50-75% within Kane, LaSalle and surrounding counties
- Proficient with standard corporate software applications (MS Word, Excel, Outlook, PowerPoint)
- Excellent communication, organizational, and interpersonal skills
Responsibilities
- Coordinate and manage casework across the continuum of care.
- Interact with members, providers, and community resources to promote wellness and appropriate benefit/return-to-work.
- Document case management activities accurately and in a timely manner.
- Collaborate with internal multidisciplinary teams to maximize health outcomes.
