CVS Health is seeking an RN Case Manager for a 100% remote role. The candidate will assess, plan, implement, and coordinate case management activities with members to evaluate medical needs and facilitate wellness. Requires active RN license in home state, 3+ years acute care nursing, and 1+ year case management experience.

CCM certification and compact licensure preferred. Travel <5% for meetings as needed.

Competenze

• Active RN license in state of residence
• 1+ year(s) case management experience
• CCM certification
• Experience with Microsoft Office (PowerPoint/Excel/Word)
• 1+ year(s) experience using computers with multiple monitors while on calls
• Associates Degree in Nursing required; Bachelor's preferred
