Job summary

CVS Health is seeking a Case Manager RN for a 100% remote role, with the employee able to reside in any state. No travel is required.

Normal hours are Monday–Friday 8:00am–5:00pm in the residence time zone, with rotational coverage of evenings, weekends and holidays as needed. Qualifications

• Active RN license in resident state
• 3+ years of acute care experience
• 3+ years of Microsoft Office experience
• Strong problem-solving and clinical decision-making
• Ability to handle sensitive information with confidentiality
• Excellent communication with diverse patients and teams

Responsibilities

• Apply data-driven methods to identify members and tailor case management programs
• Conduct comprehensive clinical assessments
• Evaluate needs and develop flexible approaches based on plans or external programs
• Advocate for patients to maximize existing health coverage
• Promote quality and cost-effective outcomes and suggest improvements
• Identify and raise quality of care issues through established channels
• Provide high level customer service
• Use assessment techniques to gauge health literacy and readiness to change
• Utilize motivational interviewing to engage members and promote healthy choices
• Provide coaching, information, education, and support to empower independent decisions
