Acentra Health is seeking a Nurse Case Manager to join our team in California. The role focuses on evaluating beneficiaries' needs, developing care plans, and coordinating the healthcare team to meet payer requirements and CMSA principles.

This remote position requires residing in California (preferably Los Angeles or San Diego area) and offers the chance to impact outcomes through integrated case and disease management, while staying aligned with HIPAA and regulatory standards.

Qualifications

• Bachelor's degree/BSN.
• Active, unrestricted RN license in CA.
• A minimum of 3+ years of clinical experience.
• A minimum of 1+ years of case management review experience.
• Must reside in the state of CA.
• Strong written and verbal communication with customer service focus.
• Ability to work independently and as part of a team.
• Maintain a high level of confidentiality.
• Ability to meet metrics and department goals.

Responsibilities

• Assess, plan, implement, monitor, and evaluate care options across the continuum of care.
• Coordinate care with beneficiaries, providers, and health team members.
• Develop beneficiary-centered care plans.
• Demonstrate shared accountability with beneficiaries and caregivers.
• Ensure timely documentation and regulatory compliance.
• Maintain HIPAA and Privacy policy adherence.
• Read, understand, and follow corporate policies.
