Job summary

Qualifications

• Bachelor's degree or BSN required.
• Active, unrestricted RN license in CA.
• Minimum 3+ years of clinical experience.
• Minimum 1+ year of case management review experience.
• Must reside in the state of CA.
• Excellent communication skills (written and verbal) and customer-service orientation.

Responsibilities

• Assess, plan, implement, monitor, and evaluate an individualized care plan across the continuum of care.
• Coordinate with the healthcare team to ensure optimal beneficiary outcomes.
• Develop beneficiary-centered care plans and maintain accountability among beneficiaries, caregivers, and providers.
• Ensure documentation and compliance with regulatory requirements and policies.
