Job summary

Acentra Health seeks a Case Manager (LPN or RN) to join our growing team in a remote role open to residents of Oregon. You will coordinate care across social, medical, and behavioral services, ensuring high-quality, compliant care planning and optimal outcomes.

You will collaborate with physicians, nurses, social workers, and OHA staff to tailor plans, educate beneficiaries, and advocate for access to community resources while maintaining PHI safeguards and regulatory standards.

Qualifications

• Current, active Oregon RN or LPN license
• 2+ years in Medicaid-funded case management
• Experience with diverse backgrounds and marginalized communities
• Training in health disparities and promoting equitable health outcomes
• Cultural competency training and ability to provide culturally appropriate care
• Experience communicating with non-English speakers or sign language users
• Experience engaging underserved communities in care planning

Responsibilities

• Provide care coordination for beneficiaries across medical, social, and behavioral services
• Collaborate with multidisciplinary care teams to meet beneficiary needs
• Address barriers impeding diagnostic, social, or treatment progress
• Educate beneficiaries about community resources and advocate on their behalf
• Complete health and social assessments using motivational interviewing and equity principles
• Maintain documentation and comply with PHI, plan of care, and regulatory standards
• Pursue ongoing education to stay current with case management standards
• Adhere to HIPAA and other privacy rules
