Job summary

Acentra Health is seeking a Nurse Case Manager to join our growing team. This role is remote, but candidates must reside in California, preferably in Los Angeles or San Diego area.

You will evaluate beneficiaries' needs, develop individualized care plans, and coordinate healthcare team members to ensure optimal client outcomes while adhering to payer requirements. CMSA principles guide case and disease management activities to stabilize health and avoid long-term care placement.

Qualifications

• Bachelor's degree/BSN required.
• Active, unrestricted RN license in CA.
• 3+ years of clinical experience.
• 1+ year of case management review experience.
• Must reside in CA.
• Strong communication and customer service.
• Ability to work independently and in a team.
• Maintain confidentiality.
• Ability to meet metrics and goals.

Responsibilities

• Assess, plan, implement, monitor, and evaluate options for individualized care across the continuum.
• Use clinical knowledge and communication skills to optimize beneficiary outcomes.
• Develop beneficiary-centered care plans.
• Maintain accountability with beneficiaries, caregivers, and providers.
• Coordinate care with the health care team.
• Address identified gaps in care or compliance.
• Protect client confidentiality and related information.
• Ensure documentation complies with regulations and policies.
