Acentra Health is seeking a Nurse Case Manager to join our growing team. The Nurse Case Manager will evaluate beneficiaries' needs, develop individualized care plans, and coordinate healthcare team members to ensure optimal client outcomes while adhering to payer requirements.

Position is remote, but candidates must reside in California, preferably in the Los Angeles or San Diego area. Responsibilities include caseload management, comprehensive documentation, and ongoing education to stay

Qualificações

• Bachelor’s degree/BSN required.
• Active, unrestricted RN license in CA.
• Minimum 3+ years of clinical experience.
• Minimum 1+ year of case management review experience.
• Must reside in CA.
• Strong communication and customer service focus.
• Ability to work independently and as part of a team.
• Maintain a high level of confidentiality.
• Ability to meet metrics and department goals.

Responsabilidades

• Assess, plan, implement, monitor, and evaluate an individualized care plan across the continuum of care.
• Develop beneficiary-centered care plans.
• Coordinate care with beneficiaries, providers, and health care teams.
• Ensure compliance with confidentiality and regulatory requirements (HIPAA).
• Maintain documentation in the web-based database accurately and timely.
