Acentra Health is seeking a Nurse Case Manager in California. The role entails evaluating beneficiaries, developing care plans, and coordinating the healthcare team to meet CMSA principles and payer requirements. Remote work is available for CA residents, with strong emphasis on documentation and regulatory compliance.

The ideal candidate holds a BSN or RN license in CA, with 3+ years clinical experience and 1+ year in case management. Bilingual in Spanish is a plus.

Formación

• Bachelor’s degree or BSN required.
• Active, unrestricted CA RN license.
• 3+ years of clinical experience.
• 1+ year of case management review experience.
• Must reside in CA.
• Strong communication and customer service skills.
• Ability to work independently and in a team.
• Maintain confidentiality at all times.
• Ability to meet departmental metrics and goals.

Responsabilidades

• Assess and plan individualized care across the continuum of care.
• Coordinate with healthcare team to ensure optimal outcomes.
• Develop beneficiary-centered care plans.
• Demonstrate accountability with beneficiaries, caregivers, and providers.
• Collaborate with beneficiary, provider, and health care team.
• Address gaps in care or compliance.
• Maintain confidentiality and HIPAA compliance.
• Document assessments accurately in the web-based database.
• Communicate openly with stakeholders.
• Pursue ongoing education and stay current with standards.
