Qualifications

• Bachelor’s degree in nursing or related years of experience, or MSW in social work preferred.
• Current RN licensure in Maryland or LCSW-C is required.
• CCM certification preferred.

Responsibilities

• Identifies and acts upon impending changes in member condition.
• Identifies and analyzes staff/member/system issues affecting care plans.
• Initiates corrective action and reviews population-specific reports.
• Maintains budgeted caseload and adheres to guidelines.
• Applies negotiation principles and uses available resources effectively.
• May assign work to support staff.
