Qualifications
- Active and unrestricted RN licensure in PA or compact license in residence.
- 3+ years of nursing experience.
- 2+ years in case management, discharge planning or home health coordination.
- Proficient with Microsoft Office Suite (Word, Excel, Outlook, Teams).
- Ability to work remotely with a dedicated space and strong collaboration.
Responsibilities
- Coordinate care telephonically for members, 40 hours weekly.
- Develop individualized care plans with goals and interventions.
- Provide disease management education to help members reach health goals.
- Involve interdisciplinary care team members as needed.
- Document care management activities in the EHR accurately.
