Qualifications

• Provide disease management education and support.
• Ensure involvement of interdisciplinary care team.
• Coordinate care to ensure a seamless health care experience.
• Document care management activity in the EHR accurately.
• Collaborate to address barriers and maintain stable health.
• Connect members with health plan benefits and community resources.
• Meet regulatory requirements within specified timelines.
• Support other care team members as needed.
