CVS Health is seeking a Case Manager RN for a 100% remote role. You will assess, plan, coordinate, advocate, and evaluate care to meet members' benefit plans and health needs.

Duties include comprehensive assessments, care coordination, discharge planning, and collaboration with Medical Directors as needed. The role requires independent judgment, strong interpersonal skills, and the ability to manage multiple priorities in a queue-driven environment.

Competenze

• Active, unrestricted RN license in home state with ability to obtain multi-state licensure on hire.
• Minimum 3 years of acute care nursing experience.
• Proficient with Microsoft Office and related tools.
• Strong problem-solving and clinical decision-making abilities.
• Excellent communication and relationship-building skills.
• Ability to handle confidential information professionally.
