Job summary

CVS Health is seeking an experienced Case Manager RN to work remotely from any state. The role focuses on assessment, planning, advocacy, and coordination to meet members' health needs and optimize outcomes, with duties spanning discharge planning and care coordination.

The position requires a licensed RN, 3+ years in acute care, proficiency with Microsoft Office, and strong communication and confidentiality skills. Hours are standard M-F with some evenings/weekends as needed.

Qualifications

• Active RN license in their state of residence; ability to obtain multi-state licensure on hire.
• 3+ years of acute care experience as an RN.
• 3+ years of experience with Microsoft Suite of products.
• Strong problem-solving and clinical decision-making skills.
• Ability to manage sensitive information with professionalism and confidentiality.
• Excellent communication and relationship-building skills.
• Ability to work effectively with diverse patient populations and healthcare teams.

Responsibilities

• Apply data-driven methods to identify members and tailor case management or referrals.
• Conduct comprehensive clinical assessments.
• Evaluate needs and develop flexible approaches based on plans and available programs.
• Advocate for patients to maximize health coverage.
• Promote quality, cost-effective outcomes and improve program efficiency.
• Identify and escalate quality of care issues through proper channels.
• Provide high levels of customer service.
• Assess health literacy and technology readiness of members.
• Use motivational interviewing to engage members and encourage healthy changes.
