Qualifikationen

• Active and unrestricted RN license in home state and ability to obtain multi-state licensure.
• 3+ years of acute care RN experience.
• 3+ years of experience with Microsoft Suite.
• Strong problem-solving and clinical decision-making skills.
• Ability to manage sensitive information with professionalism and confidentiality.
• Excellent communication and relationship-building skills.
• Ability to work effectively with diverse patient populations and healthcare teams.

Aufgaben

• Apply data driven methods to identify members for case management or referrals.
• Conduct comprehensive clinical assessments.
• Evaluate needs and develop flexible approaches based on member needs and plans.
• Advocate for patients to maximize coverage.
• Promote quality and cost-effective outcomes; suggest improvements.
• Identify and escalate quality of care issues.
• Provide high level of customer service.
• Assess health literacy and technology readiness.
• Use motivational interviewing to engage members and promote healthy changes.
• Provide coaching, information, education and support for ongoing independent medical decisions.
