Job summary

Providence is seeking a motivated RN Care Transition Coordinator to support hospitalized patients with seamless transitions from hospital to home or other care settings. The role collaborates with physicians and discharge planners, performing nursing assessments, acting as a liaison, and delivering education to patients and families while aiding the care team.

Although the role is remote, in-person training, required education, and equipment access are provided.

Qualifications

• Nursing degree required; RN license required at hire.
• Preferred: Bachelor's in nursing.
• 1 year clinical experience preferred.
• Experience in patient and staff education and conflict resolution appreciated.

Responsibilities

• Provide care transition services for hospitalized patients and coordinate with physicians and discharge planning staff to ensure seamless discharge.
• Perform nursing assessments, act as liaison, communicate effectively, educate patients and families, and support the care team.

Education

Bachelor's Degree in nursing
