Accompany Health seeks a Registered Nurse, Care Transitions to deliver seamless, high-impact care for patients with complex medical, behavioral health, and social needs. You will bridge post-acute transitions with ongoing nursing support through virtual and at-home services tailored to each patient.

You will join a multidisciplinary team to ensure continuity of care, educate patients, and empower families, all while advancing value-based, home-centered care across markets.

Qualifications

• Active RN license in state of employment (MI)
• 2+ years of experience caring for adults/geriatrics with complex conditions
• Experience in transitions of care and medication reconciliation
• Strong remote/telehealth communication and care coordination skills

Responsabilités

• Provide post-discharge transitions of care to reduce readmissions
• Complete RN visit/interaction within 7 days of discharge
• Perform clinical assessments and escalate as needed
• Coordinate post-discharge visits with AH providers within 7 days
• Maintain accurate documentation in the EMR and communicate with the care team
