Sagility is seeking a Senior Care Specialist (Registered Nurse) for a remote, work-from-home seasonal assignment ending in June 2027. The RN will conduct telephonic health assessments and develop personalized care plans for Medicare, Medicaid, and group health plan members.

Responsibilities include coordinating transitions of care, supporting education, and collaborating with a multidisciplinary team. An active RN license is required; starting pay is $37.00 per hour.

Competenze

• Active, unrestricted RN license in home state.
• Active Compact RN license permitting practice in applicable states.
• 2+ years clinical nursing experience; 3+ preferred.
• Strong clinical judgment, critical thinking, and problem-solving skills.
• Ability to work independently and in a remote environment.
• Intermediate to advanced proficiency with Word, Excel, Outlook.

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate benefits, services, and care options.
• Assess factors affecting health and wellbeing.
