Sagility is seeking a Senior Care Specialist (Registered Nurse) to perform telephonic health assessments and develop individualized care plans from a work-from-home setting in Ohio. The role emphasizes chronic condition management, care coordination, and patient education, enabling safe discharge planning and ongoing support.

You will collaborate with a multidisciplinary team, navigate multiple EMR systems, and communicate effectively with members and caregivers to promote optimal health

Qualifikationen

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Word, Excel, and Outlook.
• Ability to efficiently navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Aufgaben

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning with care coordination services.
• Help members navigate healthcare benefits, services, and care options.
• Assess factors affecting health and well-being (physical, environmental, psychosocial).
• Collaborate with multidisciplinary teams to support member care.
• Promote self-management by providing education and resources.
• Coordinate care and activities among members, caregivers, providers, and care team.
• Document interactions, assessments, plans, interventions, and outcomes timely in systems.
