The Central Transfer Center RN Care Manager coordinates safe, efficient patient transfers across the Scripps Health system. This nurse uses strong clinical judgment to assess patient needs, collaborate with physicians and care teams, and ensure placement at the appropriate facility and level of care. The role supports patient flow, timely access to services, and systemwide quality by serving as a key communication link between referring providers, internal departments, and receiving units.
As a Care Manager you will be responsible for screening patients in their assigned caseload to identify those that are medically vulnerable, who may have needs that may be addressed at another location; or within contracted programs/vendors specific to their needs/health plan. In addition, you will coordinate with the health care team and the patient/family to address issues that may impact their care, and to develop a transition plan of care based on the patients' needs.
