• Ensure continuity of care across all healthcare settings by collaborating with the interdisciplinary care team, PCP, member representatives, and specialists.
• Utilize benefit structure and community-based resources to meet member needs.
• Educate members on disease processes, preventative health, and quality of life interventions.
