Overview

We are seeking a compassionate Registered Nurse to join a growing Managed Long-Term Care (MLTC) team. In this role, you will coordinate care, conduct in-home assessments, and support older adults in living safely and independently.

Key Responsibilities

• Conduct comprehensive in-home assessments and reassessments of members’ health and functional status.
• Develop and maintain individualized care plans tailored to each member’s needs.
• Coordinate and authorize covered services across providers and payers.
• Communicate and collaborate with members, caregivers, and interdisciplinary teams to ensure optimal outcomes.
• Serve as a clinical advocate, promoting member well-being and continuity of care.
• Maintain accurate documentation and compliance with MLTC guidelines and policies.

Requirements

• Active NYS RN license & BSN
• Minimum 3 years RN experience; MLTC or geriatrics experience preferred
