  • Title: Senior Care Manager - RN
  • Duration: 6+ Months
  • Location: Remote FL Miami
  • Start Time: 10:30 AM - 07:00 PM
Description
  • Assess, reassess, and coordinate care and services.
  • Monitor person-centered care plans, educate members, and manage care.
  • Communicate with PCPs and providers, incorporating feedback into assessments and planning.
  • Mentor less experienced Care Managers.
Responsibilities
  • Conduct screenings and assessments, ensuring continuity of care.
  • Identify needs and preferences, set goals, and promote self-management.
  • Develop and monitor Person-Centered Service Plans, assisting with accommodations.
  • Manage caseload, risk stratification, and care transitions.
Requirements & Certifications
  • This is a Temp-to-Perm position.
  • This is a REMOTE role; training occurs at the local office.
  • Support a 24/7 nurse line with approximately 20-30 calls/day, mostly crisis calls.
  • Comfort with SMI required.
  • Preferred licenses: LCSW, LCPC, LMFT; Psychiatric RN acceptable with a Bachelor's degree.
  • Leadership skills for interdisciplinary teams, organizational skills, and multitasking ability.
  • Experience in Home Care, Long-Term Care, MLTC, community support services, and DME access preferred.
  • CCM or CCP certification preferred.
  • Experience in utilization review, concurrent review, or risk management is a plus.
