Job summary

CVS Health Corporation is seeking a Case Management Coordinator to support Medicare/Medicaid members from a home-based role in Illinois. You will coordinate appointments, identify and access benefits, and guide members through care management resources to improve outcomes.

The ideal candidate has 3–5 years of clinical experience, strong analytical and communication skills, and the ability to work independently in a remote setting while traveling up to 50–75% within Cook County and surrounding IL

Qualifications

• Associate degree required; Bachelor’s preferred; Registered Nurse license in Illinois active and unencumbered.
• Must reside in Illinois and have reliable transportation; up to 50–75% travel within Cook County and surrounding areas.
• Minimum 3–5 years of clinical practical experience.
• Excellent analytical and problem-solving skills; strong communication and interpersonal abilities.

Responsibilities

• Coordinate case management for members to ensure appropriate healthcare outcomes.
• Assist with appointment scheduling and access to benefits and education.
• Collaborate with members, providers, and community organizations to address health determinants.
