This role requires residence in Illinois and allows working from home with substantial travel to West side/Chicago and surrounding areas.

Qualifications

• 3–5 years clinical practical experience
• Must reside in Illinois
• Ability to travel up to 75% from home location around Chicago and surrounding areas
