Job summary

Advocate Health is seeking a full-time care manager nurse to coordinate multidisciplinary care and ensure timely, quality outcomes. The role supports patient/family engagement and works either embedded in a provider office or telephonically with a care team.

Candidates must hold an IL RN license, have a Bachelor’s in Nursing or related field, and bring 5 years of nursing experience or 1–2 years in care management. Epic EMR experience is preferred.

Qualifications

• Active IL RN license is required.
• Bachelor's Degree in Nursing or related field.
• 5 years of experience in clinical nursing or 1–2 years in care management.
• Excellent oral, written and interpersonal skills.

Responsibilities

• Coordinate care management with multidisciplinary teams to ensure timely, cost-effective care.
• Collaborate with patient/family to set SMART goals.
• Report abnormal findings and develop care plans with the care team.
• Manage CM caseload independently per department standards.
