Qualifications

Responsibilities

• Facilitate communications among patient/family and care teams to resolve barriers to timely care.
• Collaborate with primary care, hospital, and post-acute teams to ensure quality, efficient care.
• Use evidence-based approaches to engage patients in their own care and set SMART goals.
• Identify barriers to learning and report abnormal findings with plan.
• Independently manage CM caseload and documentation for regulatory requirements.
