Job summary

Molina Healthcare is seeking an experienced RN to join our remote care management team serving South Carolina. You will support care coordination across the continuum, collaborate with a multidisciplinary team, and help drive quality and cost-effective member care.

This role requires SC RN license, minimum 2 years of health care experience, and willingness to travel locally 25-40%. You will conduct assessments, develop care plans, and participate in ICT meetings to improve member outcomes.

Qualifications

• Active SC RN license required.
• Minimum 2 years health care experience, care management preferred.
• Valid driver’s license and reliable transportation for travel.
• Proficient with EMR and HIPAA compliance.
• Strong communication and time-management skills.
• Ability to work independently in diverse settings.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with clinicians and family.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor care plans, document interventions, and track outcomes.
• Maintain ongoing member caseload and proactive outreach.
• Promote integrated services across behavioral health, LTSS, and community resources.
• Facilitate interdisciplinary care team (ICT) meetings.
• Support peers with resources and recommendations as needed.
• May perform medication reconciliation when required.
